To do all these tasks in an organized manner, you need a plan that would help you to know your responsibilities and tasks as a caregiver. Plans of care documents can come in many ways.

Things you need to consider while creating your plan of care
This is one of the most important questions that we may ask ourselves, as there are several things that need to include in a care plan. Preparing the care plan by including the important and relatable tasks will help you to keep the plan simple and sorted. Including too many things, will not allow you to function properly. While preparing the care plan, you must keep in mind, that your loved one will need 24X7 support.

Always remember, a good plan of care should be in consulting with your loved one, their doctor and other professionals. Here is the list that would clearly define the tasks that you would need to take into consideration.
Involve your loved one
No care plan can be successful without the active participation of your loved one. The care plan is for the loved one, and therefore, asking them about their needs, and requirements should be the most important factor.
Consult the doctor
The care plan should include consulting with the doctor. The document would contain the medical records and records of their medications, and appointments scheduled. The doctor can also throw useful insight into what all would be best for his/her loved one.
Involve all family members
Loneliness is almost an inevitable part of old age, and while caring for your parents, you need to ensure that they don’t feel lonely and left out. Therefore, your care plan should involve the active participation of all family members.
What do we need to include in a plan of care?
A care plan should ideally keep all the aspects of the life of your loved one. The design of the care plans should be in a way that, they can make life easier and comfortable for your loved one. This can also work if your loved one is living in a care facility or a nursing home. Your care plans should be in a manner that would suggest ways, that they can enjoy their home or nursing home routine.
In order to design a care plan that best suits the needs and requirements of your loved one, you need to include the following:
Current activities of daily living, behavior, habits and routine
It is important to asses your loved one’s current situation. Observe their daily routine and also carefully make note of things, that they avoid doing. These can be simple tasks like, taking a shower, or avoiding picking up clutter, or other things. Your loved one typically avoids these tasks due to many reasons, medical or non–medical. You can even start by preparing a questionnaire for the family members. This will help you monitor which activities of daily living your loved one will need help with. Your family and friends would be the best people to answer these.
For this, you need to first understand their medical needs and document them simultaneously. This would give you a good idea about your loved one’s current status. You can go through their medical records, but make sure they are only a year old. Your loved one may suffer from multiple problems, and if need be, you can even get in touch with their doctor, to understand the situation better.
Prepare the plan of care
Now, when you have all the basic data, it is time you prepare the plan of care. Carefully note down what all you are going to do as a caregiver, and also all the routines and activities of your loved one.
Set short-term goals
Once you get started with your plan of care, you can now set short-term goals for your loved one. Write simple short-term goals, you would want your loved one to achieve. You can always involve your loved one while deciding and listing down the goals. The goals can be as simple as, doing exercises twice daily, or climbing the stairs without help, or walking at least a few steps without help or help. Compile these goals keeping in mind, the medical condition and the physical capability of your loved one. Also, consult their physician or other health care professionals seeing them for advice.
Finalizing the plan of care
Once you have designed your care plan, it is always best to discuss it with your loved one to see if they approve. There may be some areas, where your loved one might not be comfortable doing some things. In such cases, it is best to always modify things according to their likes and dislikes.
